Björn Alexandersson has initiated Team Utsocknes – a collaboration between the region and the municipality to provide more coordinated and efficient care to people with high care needs. The model, where care is provided at home, reduces the burden on the care system and improves the quality of life for the patient. A pioneering example of person-centred collaboration.

The model helps to blur organisational boundaries between municipality and region, with the potential to be scaled up nationally.
